JOB SUMMARY:

This position will provide supportive and therapeutic intervention to terminally ill patients and their families in accordance with an appropriate plan of care. The Social Worker will assume responsibility for the development, implementation, and management of psycho-social services provided to hospice patients and families.  This position participates in and facilitates interdisciplinary communication and education.

 

DUTIES & RESPONSIBILITIES:

  • Interviews patients and families in order to assess psychological, social, emotional, spiritual and economic needs.
  • Develops and implements a care plan based on assessment information and participates in patient care conferences and family meetings to review, revise and approve the patient’s Plan of Care.
  • Plans and conducts short-term goal oriented psychotherapy for patients and families as needed utilizing individual, family and group work.
  • Provides supportive and/or bereavement counseling to patients and their families to cope with illnesses and/or death.
  • Assists patient/family with necessary legal information related to end of life issues including but not limited to completing Power of Attorney for Healthcare and financial needs.
  • Makes referrals to other hospice team members (such as volunteer, Massage Therapy, Music Therapy) and /or community resources as needed for family focused holistic care including discharge planning back to community.
  • Conducts a Bereavement Assessment at time of death and develops Bereavement Plan of Care. Provides one on one bereavement follow-up as needed according to the Bereavement Plan of Care.
  • Provides patient/family with information and referral to community resources and serves as an advocate on their behalf.

 

EDUCATION/PREVIOUS WORK EXPERIENCE:

  • Master of Social Work Degree from an accredited School of Social Work.
  • Three year’s related community based experience that includes training and experience counseling individuals, families and groups.
  • One year of palliative and/hospice experience preferred
  • Eligible for registration with the Board of registration for Social Workers of the Province of BC.

 

ABILITIES/SKILLS:

  • Ability to communicate effectively in English verbally and in writing.
  • Knowledge of Systems Theory and demonstrated ability to apply Systems Theory to practice in social work assessment and intervention.
  • Knowledge of development stages in the individual and family life cycle and of psychosocial issues that may occur in response to changing health and dependency needs.
  • Ability to empathize and work with people who may have experienced abuse, poverty, grief and loss, oppression, mental health challenges, and/or addictions issues, and/or those who may be living with HIV/AIDS or other life-limiting or life-threatening conditions.
  • Knowledge of adult education principles and experience in education program development and implementation.
  • Knowledge of the mandates and policies of other government ministries such as Home and Community Care, Ministry of Social Development and the Public Guardian and Trustee.
  • Knowledge of social work theory and practice within a client and family centered model of care.
  • Knowledge of other health care disciplines and their role in client care.
  • Ability to establish rapport with clients and families in order to explore issues that are beyond the initial presented concerns.
  • Ability to function as a team member, leader, and facilitator.
  • Good working knowledge of computer programs including MS windows environment, MS Outlook, and Word.
